Abstract As the convergence of AI and the Internet of Medical Things (IoMT) continues to gain momentum, the Artificial Intelligence of Internet of Medical Things (AIoMT) paradigm has captured the attention of both academia and industry. The potential for AIoMT to revolutionize healthcare is undeniable, with applications such as alleviating the strain on healthcare systems caused by staff shortages, addressing the health challenges of aging populations, and tackling the rising incidence of chronic diseases, as well as global health crises like COVID-19. However, the dynamic nature of AIoMT and its ability to process vast amounts of patient data also make it susceptible to cyberattacks, with the potential to compromise patient data, threaten lives, and damage the reputation of medical providers. This study offers a comprehensive overview of how AI is shaping the health sector in smart cities, including the ecosystem and cybersecurity concerns, identifying the various types of cyberattacks and threats, proposing countermeasures, and exploring future developments in the field. © 2024 selection and editorial matter, Agbotiname Lucky Imoize, Valentina Emilia Balas, Vijender Kumar Solanki, Cheng-Chi Lee, Mohammad S.
