Abstract In a wide range of industries including healthcare, smart cities, agriculture and industrial automation IoT systems are utilized to carry out a number of crucial functions. The Internet of Healthcare Things (IoHT) has drawn a lot of attention as an opportunity for remote patient tracking and accuracy treatment. However, IoHT is used so extensively privacy violations like IoHT leakage of data have sparked significant concern among the public. At the contrary hand AI technologies has shown considerable promise to strengthen IoHT reliability and privacy protection. Using Artificial Intelligence (AI) techniques, the majority of these systems contain intelligence for making intelligent choices. The edge devices must be methodically built with an emphasis on the relevant AI capabilities from exploration through deployment in order to assure good performance. Technologies for healthcare has risen to the level of healthcare 5.0, which replaces earlier technologies. The objective of healthcare 5.0 is to build an entirely independent healthcare system that analyses how a patient’s many health issues interact with one another. This paper conducts a survey to determine the viability of merging AI and IoHT in protecting privacy, a comprehensive analysis of current AI-based IoHT systems is discussed. Additionally, many privacy issues in IoHT are highlighted. By addressing numerous research fields in AI, IoT and IoHT health data and security fusion, this paper review at existing research. This study also highlights some authors works to minimize the security issues by using AI technology. It additionally addresses current studies obstacles and provide some recommendations for future study. This paper concludes that there is lack of some security issues in healthcare sector which is needed to be enhanced. © The Author(s), under exclusive license to Springer Nature Switzerland AG 2025.
