Abstract Healthcare 4.0 is an e-health paradigm that emphasizes precision medicine and telemedicine to facilitate early illness detection and prediction. It is critical to modern society, particularly during pandemics. Advancements in 5G, wearable devices, artificial intelligence, edge computing, and IoT are propelling its growth, allowing it to provide healthcare services tailored to individual patients. The Internet of Medical Things (IoMT) connects smart nodes in networks to interpret, analyze, and respond to real-time data, enhancing healthcare services in smart cities. It improves service quality by enhancing connection, supercomputing, reducing latency and energy consumption and providing scalability and capacity. The combination of IoM and edge computing has become an investigative topic, aiming to manage and track IoMEC devices more efficiently. High medical urgency data is prioritized for transmission, while body sensors can monitor real-time healthcare data. However, IoMs require continuous monitoring due to limited energy and computing resources. This chapter discusses the issues and challenges associated with intelligent IoMT for edge computing and solutions to overcome these challenges, as well as various applications and use-cases. © 2025 selection and editorial matter, Hiren Kumar Thakkar, Chintan Bhatt, Victor C.M.
