Abstract In the realm of cyber-physical systems, numerous challenges must be addressed to seamlessly integrate health care within a smart city framework. This chapter provides a comprehensive overview of the hurdles and complexities inherent in implementing health-care services within a smart city context. Key issues encompass technological intricacies, human-centric considerations, legal and ethical dilemmas, infrastructure and connectivity barriers, and financial constraints. A robust network infrastructure, data security protocols, interoperability standards, and equitable access to health-care services are pivotal aspects of addressing infrastructure and connectivity challenges. Moreover, the assimilation of IoT devices, effective big data management, and grappling with the ethical implications of AI in health care exemplify the technological complexities involved. Legal and ethical considerations span regulatory compliance, data ownership, consent frameworks, and ensuring accountability. Furthermore, human factors, such as user acceptance and digital literacy, significantly influence the successful implementation of health-care initiatives in smart cities. This chapter discusses the multifaceted challenges of deploying health-care services in smart cities and emphasizes the importance of innovative strategies, resource optimization, and public–private partnerships in overcoming financial constraints. Proactive addressing of these challenges by stakeholders can lead to enhanced health-care integration, improved patient outcomes, and a more resilient health-care ecosystem in smart cities. © 2025 selection and editorial matter, Amitkumar Vidyakant Jha and Bhargav Appasani; individual chapters, the contributors.
