Abstract This study focuses on smart cities and information and communication technology in developing health services for patients to improve the quality of health services. This paper presents new ideas for solving the congestion problem, reducing waiting hours, and obtaining treatments quickly and easily for patients through smart city features. Therefore, this study aims to propose a model that is easier for patients to use, reduces waiting times, alleviates congestion, and speeds up the response. The problem of the study stems from how smart cities can improve healthcare services and whether residents face difficulties in using information technology. The case study approach was adopted for the city of Najaf using a random sample of patients, and initial interviews were conducted there. Several statistical variables were adopted through content analysis representing the study hypotheses and tried to prove them statistically. The potential methods emerging from this research are useful for city managers or large companies in the health sector that cooperate with local administration to increase the chances of long-term success of smart projects. This paper proposes a new research path integrating motivation in smart cities to achieve health service goals from a new scientific perspective. The paper emphasizes the importance of smart healthcare services within smart cities. It deals with cases or case studies specific to Najaf in Iraq and provides scientific contributions to practical applications and real-world contexts. © The Author(s), under exclusive license to Springer Nature Singapore Pte Ltd. 2025.
